I have cut 8 of them from this <a href="http://s.click.aliexpress.com/e/oPFrUMKM">12pins</a> wire terminal strip. Connect wiring: Fan1 +&-, Fan2 +&-, Hotend Thermistor +&-, Hot-end Heater +&-.
